We are looking for an experienced Band 6 Occupational Therapist to our Therapy team in delivering Intermediate Care and rapid response to patients at home or in bed based care, to prevent avoidable hospital admissions and promote self-management.

This is an exciting time to join us on a 6 month fixed term contract as we continue to develop community services in West Lancashire to ensure we are delivering the best possible service for our patients.
We are looking for a qualified and dynamic Band 6 Occupational Therapist, who can work autonomously in delivering individualised, evidence based care to patients in their own home. You will have excellent clinical skills that enable autonomous practice, the ability to work under pressure and prioritise own workload, can demonstrate clinical leadership to enable the delivery of care for patients closer to home, along with analytical and problem solving skills
